Field Trip Permission Forms
Field trip permission slips for school-sponsored excursions may be found on this page when trips are announced. Please print and fill out each form completely, then send back to your child’s teacher. St. Thomas School is unable to permit students to participate in field trips without a signed and completed permission form from a parent or legal guardian.
